El cáncer de próstata se origina en la glándula prostática. En los hombres, la próstata es una glándula pequeña con forma de nuez, que produce el líquido seminal que nutre y transporta el esperma. El de próstata es el cáncer más frecuente en hombres en el Mundo.
Aproximadamente el 3% de la población masculina puede desarrollar un cáncer de próstata después de los 50 años. Este dato que nos revela el doctor David Ruiz Londoño, urólogo oncólogo de la Clínica Medellín, es muy importante y debe alentar a todos los hombres a estar muy atentos con esta enfermedad, pues el cáncer de la próstata es el cáncer más frecuente en hombres en el Mundo.
Para entender mejor esta patología es importante destacar que el cáncer de próstata se origina cuando las células de la próstata comienzan a crecer fuera de control. La próstata es una glándula que sólo tienen los hombres. Esta glándula produce parte del líquido que conforma el semen.
La próstata está debajo de la vejiga (el órgano hueco donde se almacena la orina) y delante del recto (la última parte de los intestinos). Justo detrás de la próstata se encuentran las glándulas llamadas vesículas seminales, las cuales producen la mayor parte del líquido del semen. La uretra, que es el conducto que transporta la orina y el semen fuera del cuerpo a través del pene, pasa por el centro de la próstata.
El tamaño de la próstata puede cambiar a medida que el hombre envejece. En los hombres más jóvenes, la próstata es del tamaño aproximado de una nuez. Sin embargo, puede ser mucho más grande en hombres de más edad.
El doctor Ruiz destaca que muchos cánceres de próstata crecen lentamente y permanecen limitados a la glándula prostática, en donde pueden no causar daño grave. Pero, si bien algunos tipos de cáncer de próstata crecen lentamente y pueden necesitar un tratamiento mínimo o no necesitar ningún tratamiento, otros tipos son agresivos y se pueden diseminar con rapidez.
Detección temprana
Igualmente, nuestro invitado pondera que el cáncer de próstata que se detecta en forma temprana, cuando todavía está limitado a la glándula prostática, tiene mejores probabilidades de tener un tratamiento exitoso.
“El cáncer de próstata en la mayoría de las ocasiones es asintomático y generalmente lo queremos buscar asintomático. La mayoría de los pacientes no sienten absolutamente nada y cuando se torna sintomático es que ya tiene un cáncer muy avanzado y allí el pronóstico definitivamente es más complicado”, puntualiza nuestro invitado.
Dificultad para orinar, sangre en la orina o en el semen, dolor al eyacular o miccionar, flujo de orina débil o interrumpido, micción frecuente, en especial por las noches, son algunas de las señales que deberían alertarnos de una posible presencia de un cáncer de próstata.
Tipos de cáncer de próstata
Casi todos los cánceres de próstata son adenocarcinomas. Estos cánceres se desarrollan a partir de las células glandulares (las células que producen el líquido prostático que se agrega al semen).
Otros tipos de cáncer que se pueden originar en la próstata son:
- Carcinomas de células pequeñas
- Tumores neuroendocrinos (aparte de los carcinomas de células pequeñas)
- Carcinomas de células transicionales
- Sarcomas
Estos otros tipos de cáncer de próstata son poco comunes. Si a usted le dicen que tiene cáncer de próstata es casi seguro que sea un adenocarcinoma.
Factores de riesgo
De la misma forma, el doctor Ruiz destaca que en los hombres hay un factor de riesgo muy importante y es el familiar, una persona que tenga un hermano, un papá, un abuelo con cáncer de próstata tiene un riesgo mayor de tener un cáncer de próstata que una persona que no tenga historia familiar.
Normalmente la recomendación para los hombres es comenzar a hacerse exámenes a los 50 años, pero para estas personas con antecedentes familiares de cáncer de próstata es recomendado iniciar desde los 45 años.
Otros factores que pueden aumentar el riesgo de cáncer de próstata son la edad avanzada (es más frecuente después de los 50 años), la raza (por razones que aún no se han determinado, los hombres afroamericanos tienen un mayor riesgo de cáncer de próstata que los de otras razas), y la obesidad.
El tacto rectal
El doctor Ruiz señala con gran preocupación que muchos hombres no acuden a consulta por temor a que se les haga un tacto rectal, pero puntualiza que hacer tacto rectal no hace parte de la obligatoriedad de la consulta con el urólogo, “la mayoría de las veces si el antígeno está normal no hay necesidad de hacerle el tacto rectal a los a los pacientes”, enfatiza.
El antígeno, explica nuestro invitado, se evalúa a través de la prueba del antígeno prostático específico (PSA), la cual se efectúa a través de la extracción de una muestra de sangre de una vena del brazo. Esta es una prueba que mide la concentración del PSA en la sangre. El PSA es una sustancia que se produce en su mayor parte en la próstata; a veces se encuentra en mayor cantidad en la sangre de los hombres que tienen cáncer de próstata y es en esos casos que se requiere evaluaciones y exámenes más específicos.
Recomendaciones
“No me canso de decirlo, la más importante recomendación que puedo hacerle a mis pacientes y a los hombres en general es hacerse la prueba de antígeno desde los 50 años”, enfatiza nuestro invitado.
Otras recomendaciones son seguir una dieta saludable con muchas frutas y verduras, elegir alimentos saludables en lugar de tomar suplementos, hacer ejercicio la mayoría de los días de la semana, mantener un peso saludable y consultar con tu médico en caso de tener algún síntoma.
